Volvulación gástrica aguda. Hallazgos en técnicas de imagen.

Resumen

El vólvulo gástrico es una patología infrecuente que ocurre cuando se produce una rotación anormal del estómago mayor de 180º y sobre uno de sus dos ejes. Se puede presentar de forma aguda con dolor abdominal y vómitos o bien como diagnóstico incidental si es crónico y la sintomatología es silente. Su diagnóstico se puede realizar mediante endoscopia digestiva o bien prueba de imagen, siendo la tomografía computarizada de gran ayuda para descartar posibles complicaciones como la isquemia intestinal o infarto. El tratamiento es variable: en pacientes de alto riesgo quirúrgico se suele apostar por la endoscopia, en cambio, la gastropexia conformaría el tratamiento definitivo con menor riesgo de recidiva.

Abstract

Gastric volvulus is a rare pathology that occurs when there is an abnormal rotation of the stomach greater than 180º and on one of its two axes. It can present acutely with abdominal pain and vomiting or as an incidental diagnosis if it is chronic and the symptoms are silent. Its diagnosis can be made by digestive endoscopy or imaging test, being computed tomography of great help to rule out possible complications such as intestinal ischemia or heart attack. Treatment is variable: in patients with high surgical risk, endoscopy is usually used, while gastropexy would be the definitive treatment with a lower risk of recurrence.
