Colecistitis aguda enfisematosa. Una entidad a tener en cuenta en el diagnóstico diferencial de dolor abdominal agudo.

Resumen

La colecistitis aguda enfisematosa es una entidad infrecuente que se caracteriza por la presencia de gas en la pared y/o la luz vesicular. Su morbimortalidad es muy elevada por su curso rápidamente progresivo hacia la sepsis, el shock e incluso el exitus si no se lleva a cabo una colecistectomía emergente. Dado que el cuadro clínico inicial no es específico, pudiendo ser similar al de la colecistitis aguda, es crucial tener un alto índice de sospecha.

Presentamos el caso de un paciente que ingresó por dolor abdominal agudo y que requirió la realización de tres pruebas de imagen urgentes en un breve periodo de tiempo hasta poder alcanzar el diagnóstico de colecistitis aguda enfisematosa.

Abstract

Acute emphysematous cholecystitis is a rare entity characterised by the presence of gas in the gallbladder wall and/or lumen. Its morbidity and mortality is very significant due to its rapidly progressive course towards sepsis, shock and even exitus if an emergency cholecystectomy is not performed. As the initial clinical presentation is not specific and may be similar to that of acute cholecystitis, it is crucial to have a high index of suspicion.

We present the case of a patient who was admitted for acute abdominal pain and required three urgent imaging tests in a brief period of time to achieve a diagnosis of acute emphysematous cholecystitis.
