Hepatitis autoinmune: etiopatogenia, diagnóstico y tratamiento.



Resumen

La hepatitis autoinmune es una enfermedad de curso insidioso que se define como la inflamación crónica del parénquima hepático de etiología desconocida. En su etiopatogenia parecen influir desencadenantes medioambientales y falta de tolerancia en pacientes genéticamente predispuestos que conllevan el ataque del parénquima hepático por parte de linfocitos T. Para su diagnóstico se requieren características histológicas y analíticas específicas, entre las que destacan hipergammaglobulinemia y la positividad para ciertos autoanticuerpos, que la clasifican en tipo I (ANA y/o ASMA y/o SLA/LP) y tipo II (LKM-1 y/o LC1). Dado que en rara ocasión la hepatitis autoinmune entra en remisión, el principal objetivo de su tratamiento es modificar su historia natural, además de aliviar la sintomatología, mejorar los parámetros bioquímicos y disminuir el componente inflamatorio y por tanto la fibrosis del tejido hepático.

Autoimmune hepatitis is a disease of insidious course which is defined as the chronic inflammation of the liver parenchyma of unknown etiology.

It seems that environmental factors and lack of tolerance influence its etiopathogenesis in patients genetically predisposed carrying out the attack of the liver parenchyma by T lymphocytes. Specific histological and analytical features are necessary for its diagnosis, among which hypergammaglobulinemia and positivity for certain autoantibodies stand out, which classify it as type I (ANA and/or ASMA and/or SLA/LP) and type II (LKM-1 and/or LC1). Given that autoimmune hepatitis rarely goes into remission, the main goal of the treatment is to modify its natural history, in addition to relieving its symptomatology, improving the biochemical parameters and decreasing the inflammatory component and therefore the fibrosis of the liver tissue.
