Síndrome de Bouveret: casos clínicos y revisión en la literatura.

Resumen

El síndrome de Bouveret constituye la variedad más infrecuente de íleo biliar y se describe en la literatura médica como una entidad poco frecuente. Las manifestaciones clínicas son inespecíficas y el diagnóstico puede ser tanto endoscópico como radiológico para detectar el cuadro de obstrucción intestinal de forma precoz y ser lo más conservadores posibles, siendo de elección la extracción endoscópica y si ésta fracasa la cirugía, controvertido es, si se debe realizar la cirugía en uno o dos tiempos. A pesar de todo, la mortalidad en estos pacientes es elevada ya que afecta preferentemente a pacientes ancianos con numerosas comorbilidades, de ahí que consideremos imperativo intentar solucionar el problema mediante técnicas menos agresivas.

Presentamos cuatro casos clínicos de reciente diagnóstico en nuestro hospital, su manejo y tratamiento tras la revisión del tema en la literatura.

Bouveret syndrome is the rarest variety of gallstone ileus and is described in medical literature as a rare entity. Clinical manifestations are nonspecific and diagnosis can be both endoscopic and radiological in order to detect bowel obstruction at an early stage and be as conservative as possible. The endoscopic removal of the gallstone is the technique of choice and if it fails surgery is performed. It is controversial whether surgery should be performed in one or two stages. Nevertheless, mortality in these patients is high as it mainly affects elderly patients with numerous comorbidities, hence we consider imperative to try to solve the problem through less invasive techniques.

We present the cases of four patients recently diagnosed in our hospital, as well as their management and treatment after review of this subject in medical literature.
