Resumen

Introducción

M2-Piruvato Quinasa fecal (M2-PK) es un nuevo biomarcador útil para diagnosticar la enfermedad inflamatoria intestinal.

Objetivos

Determinar la validez diagnóstica de M2-PK fecal para predecir actividad endoscópica y clínica en la enfermedad de Crohn (EC) comparado con calprotectina, PCR y VSG.

Material

Estudio de validación de prueba diagnóstica de 58 pacientes con EC determinándose SES-CD, CDAI, M2-PK, Calprotectina, PCR y VSG. Calculamos correlación y curvas ROC de los biomarcadores estimando la sensibilidad y especificidad óptima para establecer actividad clínica y endoscópica.

Resultados

M2-PK y calprotectina mostraron diferencias significativas (p<0.05) al comparar media de los grupos en remisión y actividad para SES-CD y CDAI. PCR y VSG no presentaron diferencias significativas (p>0.05) para SES-CD pero sí para CDAI. La correlación con SES-CD y CDAI presentó valores significativos para M2-PK (r=0.48 y r=0.25), Calprotectina (r = 0.70 y r = 0.54), PCR (r = 0.52 y r = 0.58) y VSG (r = 0.40 y r = 0.45). El área bajo la curva (AUC) respecto al SES-CD y CDAI fue significativo para M2-PK (AUC = 0.78 y AUC = 0.25), Calprotectina (AUC = 0.89 y AUC = 0.83), PCR (AUC = 0.69 y AUC = 0.80) y VSG (AUC = 0.66 y AUC = 0.72). M2-PK y calprotectina presentaron sensibilidad 85% y especificidad 78% para establecer actividad endoscópica. M2-PK presentó sensibilidad 67% y especificidad 65% y calprotectina sensibilidad 79% y especificidad 76% para determinar actividad clínica.

Conclusiones

M2-PK fecal es un biomarcador válido y con alta precisión para predecir actividad endoscópica en la EC siendo superiores a PCR y VSG. M2-PK fecal es válido para definir actividad clínica pero menos preciso que calprotectina fecal, PCR y VSG. Calprotectina fecal presenta mayor precisión diagnóstica que M2-PK, PCR y VSG para predecir actividad clínica y endoscópica en la EC.

Introduction

Fecal M2-pyruvate kinase (M2-PK) is a new useful biomarker to diagnose inflammatory bowel disease.

Aims

To determine the diagnostic accuracy of fecal M2-PK to predict clinical and endoscopic activity in Crohn’s disease (CD) compared to calprotectin, CRP and ESR.

Material

Validation study of a diagnostic test for 58 patients with CD determining SES-CD, CDAI, M2-PK, calprotectin, CRP and ESR. We calculated the correlation and ROC curves of biomarkers estimating the optimal sensitivity and specificity to establish clinical and endoscopic activity.

Results

M2-PK and calprotectin showed significant differences (p <0.05) when comparing the groups average in remission and activity for SES-CD and CDAI. CRP and ESR did not differ significantly (p> 0.05) for SES-CD but they did for CDAI. The correlation with SES-CD and CDAI showed significant values for M2-PK (r = 0.48 and r = 0.25), calprotectin (r = 0.70 and r = 0.54), CRP (r = 0.52 and r = 0.58) and ESR (r = 0.40 and r = 0.45). The area under the curve (AUC) compared to the SES-CD and CDAI was significant for M2-PK (AUC = 0.78 and AUC = 0.25), calprotectin (AUC = 0.89 and AUC = 0.83), CRP (AUC = 0.69 and AUC = 0.80) and ESR (AUC = 0.66 and AUC = 0.72). M2-PK and calprotectin showed 85% sensitivity and 78% specificity to establish endoscopic activity. M2-PK showed 67% sensitivity and 65% specificity and calprotectin showed 79% sensitivity and 76% specificity to determine clinical activity.

Conclusions

Fecal M2-PK is a valid biomarker with high accuracy to predict endoscopic activity in CD with better results than CRP and ESR. Fecal M2-PK is valid to define clinical activity but less accurate than fecal calprotectin, CRP and ESR. Fecal calprotectin has higher diagnostic accuracy than M2-PK, CRP and ESR for predicting clinical and endoscopic activity in CD.
