Hepatocarcinoma avanzado: ¿qué hay más allá del sorafenib?

Advanced hepatocellular carcinoma: wthat is there beyond sorafenib?


Resumen

El hepatocarcinoma (CHC) es la segunda causa de muerte por cáncer a nivel mundial y la octava en España. La historia natural del CHC es muy variable, siendo su principal factor de riesgo la cirrosis hepática y el BCLC el sistema más estandarizado actualmente para su estadiaje y tratamiento. El diagnóstico de un paciente con CHC avanzado puede presentarse por recurrencias de estadios más precoces o por deficiencias en los programas de cribado. El sistema BCLC ofrece diferentes opciones de tratamiento para estadios más precoces del CHC; sin embargo, en estadios más avanzados disponemos aún de pocas opciones de tratamiento y hay cuestiones sin resolver. El sorafenib, un inhibidor multiquinasa, es el único tratamiento aceptado para el CHC avanzado siendo muchas las líneas de investigación y moléculas en estudio para el tratamiento de pacientes que no responden al mismo. En esta revisión intentaremos hacer un repaso de todas estas cuestiones.

Abstract

Hepatocarcinoma (HCC) is the second cause of cancer death worldwide and the eighth in Spain. The natural history of HCC is highly variable, with the main risk factor being cirrhosis and BCLC, the most standardized system for staging and treatment. The diagnosis of a patient with advanced HCC may be due to recurrences of earlier stages or deficiencies in screening programs. The BCLC system offers different treatment options for earlier CHC stages; however in more advanced stages we still have few treatment options and there are unresolved issues. Sorafenib, a multikinase inhibitor, is the only treatment accepted for advanced HCC with many lines of research and molecules under study for the treatment of patients who do not respond to sorafenib. In this review we will try to review all these issues.
