Malabsorption and intolerance to lactose, fructose and sorbitol diagnosed through hydrogen breath test: epidemiology and prevalent symptoms.


Resumen

Introducción y objetivos. La malabsorción e intolerancia a los hidratos de carbono producen síntomas inespecíficos, siendo el test de hidrógeno espirado (THE) el método diagnóstico de elección. El objetivo del estudio fue conocer las características clínicas y epidemiológicas de esta patología.

Material y métodos. Estudio observacional, descriptivo y retrospectivo de pacientes que se realizaron el THE en el Hospital de Jerez de la Frontera entre julio de 2017 y diciembre de 2018.

Resultados. Se incluyeron 353 pacientes, sin hallarse asociaciones significativas entre edad, sexo e IMC con la presencia de malabsorción o intolerancia. La malabsorción más frecuente fue de lactosa (30,2%), así como los grados leves (I). Fue estadísticamente significativa la asociación entre malabsorción de fructosa y sorbitol. Con respecto a las intolerancias, también fue la más frecuente a la lactosa (17,8 % de los pacientes), siendo estadísticamente significativas las asociaciones entre intolerancia a lactosa y fructosa; e intolerancia a fructosa y sorbitol. Los síntomas digestivos más frecuentes por los que se indicó el THE fueron la diarrea (42,9%), el dolor abdominal (29,1%) y la distensión abdominal (20,3%), sin existir asociación significativa de que alguno fuera un síntoma predictor de un THE positivo.

Conclusiones. La malabsorción y la intolerancia a lactosa, fructosa y sorbitol son frecuentes en nuestro medio siendo causantes de síntomas digestivos que con frecuencia son motivos de consulta. Disponemos de un método sencillo de diagnóstico, pero no de síntomas predictivos que sean determinantes de esta patología.

Abstract

Introduction and aims: Carbohydrate intolerance and malabsorption have a nonspecific clinical appearance. The Hydrogen Breath Test (or HBT) is the chosen method of diagnosis. The objective of this research was to know the clinical and epidemiological characteristics of this pathology.

Material and methods: Retrospective, descriptive and observational study about patients that underwent the HBT in the Hospital de Jerez de la Frontera between July 2017 and December 2018.

Results: 353 patients were included, without any significant relationship between age, gender and BMI, and the presence of malabsorption or intolerance. The most common malabsorption was lactose (30,2%), being more frequent mild levels (I). It was especially significant the relationship between fructose and sorbitol malabsorption. Lactose intolerance was the most common (17,8% of the patients), being associated between lactose intolerance and fructose, as sorbitol intolerance and fructose were statistically significant. The most frequent digestive symptoms that led to the HBT were diarrhoea (42,9%), abdominal pain (29,1%) and abdominal bloating (20.3%), but there is not a significant association indicating that any of them is an indicator symptom of a positive HBT.

Conclusions: Lactose, fructose and sorbitol malabsorption and intolerance are common in our setting, causing digestive symptoms that are often reasons for consultation. We have a simple method of diagnosis, but no predictive symptoms that are determinants of this pathology.
