Síndrome de Lemmel: una causa infrecuente de ictericia obstructiva intermitente.

Lemmel syndrome: a rare cause of intermittent obstructive jaundice.


Resumen

El síndrome de Lemmel consiste en un cuadro de ictericia obstructiva secundaria a un divertículo periampular en ausencia de otras causas de obstrucción, que se acompaña de dolor abdominal o incluso colangitis aguda. La mayoría de los pacientes con síndrome de Lemmel presentan ictericia, dolor abdominal, colangitis o pancreatitis aguda, pudiendo estos síntomas ser intermitentes. Su diagnóstico puede hacerse mediante visualización directa en la colangiopancreatografía retrógrada por endoscopia o bien por imagen, utilizando la TC y la RM por su capacidad multiplanar.

El tratamiento no está justificado en casos asintomáticos. En cambio, en presencia de síntomas, la esfinterotomía mediante CPRE es el tratamiento de elección.

Abstract

Lemmel syndrome consists of obstructive jaundice secondary to a periampullary diverticulum in the absence of other causes of obstruction, accompanied by abdominal pain or including acute cholangitis. Most patients with Lemmel syndrome present with jaundice, abdominal pain, cholangitis, or acute pancreatitis, and these symptoms may be intermittent. Its diagnosis can be made by direct visualization in endoscopic retrograde cholangiopancreatography or by imaging, using CT and MRI due to its multiplanar capacity.

Treatment is not justified in asymptomatic cases. In return, in the presence of symptoms, ERCP sphincterotomy is the treatment of choice.
