Obstrucción de prótesis metálica autoexpandible cubierta por el ingrowth generado tras la pérdida de del recubrimiento de la malla en paciente con pancreatitis crónica

Resumen

Las estenosis biliares benignas en pacientes con pancreatitis crónicas resultan difíciles de tratar y presentan altas tasas de recurrencia. Las prótesis plásticas múltiples y las prótesis metálicas autoexpandibles (PMA) totalmente cubiertas se han mostrado igualmente eficaces y seguras. La adherencia a los recambios protésicos puede verse afectada en esta población de pacientes cuando continúan con un consumo abusivo de alcohol. Presentamos un inusual caso de colangitis aguda por obstrucción de una PMA totalmente cubierta por el ingrowth generado tras la desintegración de la cubierta de la malla a los cuatro años de su colocación.

Abstract

Benign biliary strictures in patients with chronic pancreatitis are difficult to treat and have high rates of recurrence. Multiple plastic prostheses and fully covered self-expanding metal stents (SEMS) have been shown to be equally effective and safe. Adherence to prosthetic replacements may be affected in this population of patients when they continue with an abusive consumption of alcohol. We present an unusual case of acute cholangitis due to obstruction of a self-expanding metal stent completely covered by the ingrowth generated after the disintegration of the mesh cover four years after its placement.
