Apendicitis aguda secundaria a tumor carcinoide



Resumen

Los tumores carcinoides apendiculares son entidades poco frecuentes, aparecen con más frecuencia en el sexo femenino entre la cuarta y quinta década de la vida. Se diagnostican mediante el estudio histológico de la pieza quirúrgica siendo en ocasiones un hallazgo incidental. La extensión de la resección quirúrgica a nivel del apéndice cecal depende del tamaño tumoral. Presentamos el caso de una paciente de 31 años de edad que acudió a urgencias por presentar dolor en región epigástrica irradiado a fosa iliaca derecha y resto del abdomen sin signos de peritonismo, presentaba neutrofilia, aumento de reactantes de fase aguda y ligera elevación de enzimas hepáticas. Las técnicas de imagen diagnosticaron apendicitis aguda y tras el análisis histológico de la pieza quirúrgica se evidenció la existencia de un tumor carcinoide a nivel del apéndice cecal.

Appendiceal carcinoid tumors are uncommon entities, and they occur most frequently in women between the fourth and fifth decade of life. They are diagnosed by histology of surgical specimens, being sometimes an incidental finding. The extent of surgical resection at cecal appendix level depends on tumor size. We present the cas of a 31-year-old woman who went to the emergency services with epigastric pain radiating to the right iliac fossa and the rest of the abdomen without signs of peritonitis. She also showed neutrophilia, an increase in the concentration of acute phase reactants and a slight elevation of liver enzymes. Through the use of imaging techniques the patient was diagnosed with acute appendicitis and the histological examination of the surgical specimen showed the existence of a carcinoid tumor of the appendix.
